The Rehabilitation Program is ideal for those patients who no longer require an acute care hospital but who are unable to return home immediately following hospitalization. Our case management team works with each patient and their family to establish a treatment program that is based on individual needs and goals. This team incorporates doctors, nurses, therapists, and dietary specialists who continuously monitor, evaluate, and modify treatment plans and goals as part of the patient’s recovery to return home. |

Comprehensive discharge planning for your return to home and community begins upon your arrival at Astoria Park. Through our home management program, we help ease the transition from rehabilitation to independence. Our spacious therapy suite is equipped with a working kitchen and bathroom where you can build and practice your daily living skills before going home.
Our therapy and social services professionals will evaluate your home environment to assess the special challenges you'll face there. If necessary, we will recommend adaptive equipment or modifications to make home life more comfortable. We'll introduce you to community services to further assure your continued well-being. We also offer instruction and guidance to your family members so they can better assist you upon your return home.

Fully licensed therapists at Astoria Rehabilitation Center provide highly individualized rehabilitation services to decrease pain, increase strength and flexibility, and maximize independence. Our treatment modalities comprise: electric simulation, extensive therapeutic exercise program, gait training, ultrasound and hydrotherapy.
